Why Medication Errors Occur 

A study published in the medical journal, Pediatrics, found that hospitals erroneously administered medications to children 11% of the time and alarmingly, these errors could have been prevented 22% of the time. Medication errors can be much more harmful to children than adults because of their smaller size and their organs are not as fully formed as adults. This means their bodies are not properly equipped to deal with even small variances in medications and these errors can lead to detrimental and serious issues. 

A study done by Medmarx, an anonymous national reporting database operated by the U.S. Pharmacopeia (USP), found that hospital medication errors occurred because doctors and nurses are often distracted, dealing with workload increases, and deal with staffing issues. Medmarx also found that communication errors and lack of physician follow-up after medication distribution put patients in harm’s way.
