  • Falls (36.9%) – Common factors include unprotected sides, wall openings, floor holes, misuse of ladders, protruding steel bars, and improper scaffolding construction.
  • Struck by an object (10.3%) – Common factors include struck by a vehicle and falling/flying objects.
  • Electrocutions (8.9%) – Common factors include improper use of cords, contact with power lines, and lack of ground-fault.
  • Caught between objects (2.6%) – Common factors include lack of protective systems, failure to inspect trenching systems, unsafe spoil-pile placement, and unsafe access/egress.
