There is a way to tell the age of a herniated disk by examining an MRI scan. Specifically, a stress test MRI shows images of the herniated disk. These injuries do start with the soft, gelatinous inner cartilage protruding out of the tough outer casing of the disk. However, with time, these soft inner materials dry out and harden the longer that they remain outside of the disk.
The stress test MRI takes an image of the spine while it is upright and another image of the spine while the person is lying down. The doctor can compare these two images by looking at how much of the inner gel is protruding to determine the age of the injury. When the upright position shows that the gel is protruding even more due to the pressure that the spine is bearing, it shows that the injury is more recent. In this case, the lying down position should show some of the gel go back into the disk. If the injury is older, then the gel will not be able to squeeze out as much since it is hardened and cannot move as much as the soft gel from a recent injury can. The images of older injuries will not differ as much.
